This article demonstrates how to utilize note Templates for tracking a crown lab, but it can be expanded to include dentures, space maintainers, retainers, bridges etc. by adding additional components to the Question or Template. The examples demonstrated in this article are meant to be used as a guide, and are intended to be customized by each office to fit their specific lab case needs.

Steps

Section 1 - Adding a Lab Case Question Category

1. Click the Application button in the upper left corner, and click Curve Forms.

   

2. Curve Forms opens. Click the Note Templates icon.

   

3. In the Note Templates module, click Questions.

   

4. Click Add Question Category to add an additional category to the list.

   

Section 2 - Creating a Lab Case Question

1. In the Question Category Name field, type Lab Cases and click Save.

   

2. Click the More options menu for the new category and click Add Question to Category.

   

3. Type Lab Case Slip in the Question Name field.

   

4. Drag and drop the Dropdown component from the Toolbox into the builder.

   

5. In the Answer tab, enter the dental labs used by the clinic.

   

Section 3 - Creating a Conditional Question for Other Lab

1. Add a Short Answer component labeled Lab name.

   

2. On the Conditional tab, set the component to display as True when Lab where case was sent "Has the value" of Other.

   

Section 4 - Adding a Repeater Component

1. Drag a Repeater component into the builder and label it Crown information.

   

2. Drag Site and Short Answer (labeled Shade) components into the Repeater box to allow for multiple entries.

   

Section 5 - Adding Additional Components

1. Add a Material component and filter the answers to only show relevant crown materials.

   

2. Add another Short Answer labeled Due Date and Time and click Save.

   

Section 6 - Adding the Lab Case Question to a Template

1. Switch to Templates and create a category named Lab Case.

   

2. Add a new template, then drag the Lab case slip question into the builder.

   

3. Add a Checkbox component for Providers to allow selection of the treating doctor.

   

Section 7 - Utilizing a Lab Case Note Template

1. In Charting, right click a visit and select Add templated note.

   

2. Fill out the template details and click the checkmark.

   

3. The note displays on the Treatment Plan Card. You can right click to Edit or Print it.
