This article demonstrates how to create a question that can be used in Note Templates to document a patient's behavior during their appointment. The question created in this article, or a similar question, can be utilized for multiple types of Note Templates.
About
Questions are used as the building blocks of Note Templates. Create a set of questions that fit the specific needs of the patients and appointments for your clinic rather than following this example exactly as it is laid out. This is intended to be a guide only, and all the question components added are suggestion only.

There are various ways to create Questions for building Note Templates. This article demonstrates some options, but the office can select the question components they prefer. It can be helpful to open an existing QuickText template for fillings in Curve Hero to get ideas for creating the Questions in Curve Forms.
Understanding Conditional Questions
A question can be set up so that if an option is selected, another set of options appears to clarify or give additional information. A conditional question contains multiple components. The first component will be called the primary question in this article. The other components will be set up conditionally so they appear upon the selection of specific values of the primary question.

Steps
Section 1 - Creating a Question Category

In Curve Hero, click the Application switcher drop down menu, and click Curve Forms.

In the Sidebar click the Note Templates icon.

Click Questions to open the Questions module.

To create a new category, click Add Question Category.
Note: If the category has already been created, proceed to Section 2.

Type the category name and click Save.

Section 2 - Adding a Question for Patient Behavior During Appointment

Click the More Options menu on the category row and select Add Question to Category.

Enter the Question Name for this component.

Drag the Section Header from the Tool Box and drop it into the builder. Click Edit to label it "Patient behavior".

Drag the Radio component under the header. Click Edit to set the label and add behavior options (e.g., Cooperative, Anxious) in the Answer tab.

Section 3 - Adding a Conditional Component

Drag a Select boxes component into the builder. Click Edit and label it "Patient requires". Add options in the Answer tab.

Return to the primary question's Answer tab and copy the Value field for the trigger answer (e.g., Anxious).

Edit the secondary component's Conditional tab. Set "This component should display" to True when the primary component "Has the value" pasted from the clipboard.

Repeat this process for other answers requiring clarification, such as "Noncompliant".

Click Save to store the question with all components.
