This article demonstrates how to create question components for a Note Template that can be used for a patient's regular hygiene appointment.
About
Questions are used as the building blocks of Note Templates. Create a set of questions that fit the specific needs of the patients and appointments for your clinic rather than following this example exactly as it is laid out. This is intended to assist and give ideas so the end result fits the needs of the hygiene program of the office.

It can be helpful to open an existing QuickText template for a regular hygiene visit in Curve Hero to get ideas for creating the Questions in Curve Forms.

Section 1 - Creating a Question Category

In Curve Hero, click the Application switcher drop down menu, and click Curve Forms.

In the Sidebar click the Note Templates icon.

Click Questions to open the Questions module.

To create a new category, click Add Question Category.
Note: If the category exists, proceed to Section 2.

Type the category name and click Save.

Section 2 - Creating a Question for a Hygiene Note Template

Click the More options menu on the category row and click Add Question to Category.

Type the name for this question in the Question Name field.

Drag and drop a Section Header from the Tool Box.

Add a Dropdown component, click Edit, and label it Performed. Add "Adult prophy" and "Child prophy" in the Answer tab.

Section 3 - Building Prognosis Components

Add the Excellent, Good, Fair, Poor component and label it Home care.

Add the Radiographs component and use the Answer tab to add "PA" and "FMX" options.

Section 4 - Creating a Conditional Component

Add a Short Answer component labeled Site(s).

In the Conditional tab, set "This component should display" to True when the form component Radiographs "Has the value" of Periapical.

Section 5 - Building a Component for Bleeding Gums

Add the Mild, Moderate, Severe component and label it Calculus.

Add the Generalized, Localized component and label it Does the patient have bleeding gums?. Add a "No bleeding" option in the Answer tab.

Add a Text box component labeled Additional notes. Click Save in the bottom right corner when the question is complete.
