This article demonstrates how to build a Question for Administered Anesthesia that can be used in any applicable Note Template.
About
Questions are used as the building blocks of Note Templates. Create a set of questions that fit the specific needs of the patients and appointments for your clinic rather than following this example exactly as it is laid out. This is intended to assist and give ideas so the end result fits the specific types of anesthesia used in the office. All question components in this article are suggestions only, and are meant to be customized.

Section 1 - Creating a Question Category

In Curve Hero, click the Application switcher drop down menu, and click Curve Forms.

In the Sidebar click the Note Templates icon.

The Note Templates section opens. Click Questions to open the Questions module.

To create a new category, click Add Question Category.
Note: If the category has already been created, proceed to Section 2.

Type the category name and click Save.
Section 2 - Add a Dropdown component for Topical Anesthesia

On the designated Category row, click the More Options menu and select Add Question to Category.

The Question Builder page opens. Enter the Question Name as it will appear under the category.

In the Tool Box, drag and drop the Dropdown component into the Question Builder section.

Click the Edit icon on the component and set the Label to Topical anesthesia.

In the Answer tab, enter the types of topical anesthesia used in the office (e.g., Benzocaine, Cetacaine) and click Ok.
Section 3 - Adding Components for Local Anesthesia

Drag and drop the Select Boxes component into the builder and label it Local anesthesia injected.

Use the Answer tab to add all anesthesia types commonly administered in the office (e.g., Septocaine, Lidocaine).

Drag a Short Answer component into the builder and label it Additional notes and reactions.

Click Save to store all components for this question. The question is now ready to be added to Templates.
