This article demonstrates how to create a Note Template in Curve Forms that can be utilized to fill out patient note information for a fillings appointment.
About
Note Templates are different than Questions. Questions can be created first, which can make the process of creating Note Templates much easier. The examples used in this article are done by use of Questions that are already created. Create a set of Note Templates that fit the specific needs of the patients and appointments for your clinic rather than following this example exactly as it is laid out. All questions and note components in this article are suggestions only, and are meant to be customized.

Section 1 - Creating a Note Template Category

Note Templates are created in Curve Forms. Click the Application Switcher button, then click Curve Forms.

In the left sidebar, click the Note Templates icon.

There are two options from which to select: Questions and Templates. Click Templates.

Create a note category for the note template. Click Add Template Category.

Type the name for this category and click Save.

Section 2 - Creating a Note Template with Questions
IMPORTANT NOTE: All components used in this section will need to be created as "Questions" before they will display in the template menu.

Click the More options menu on the desired category row. Click Add Template to Category.

Type the template name and ensure the Color matches the corresponding Shortcut Button in Charting.

Pull previously created questions from the Template menu into the Template Builder section.

Find the Fillings category and drag the component into the builder.

Add a Dropdown from the Tool Box for "Material" and enter your office's standard options (Composite, Amalgam, etc.).

Drag and drop other questions like Anesthesia, Provider, and Patient Behavior into the builder.

Section 3 - Adding Additional Note Template Components

Use the Tool Box to add components directly to a specific template if they are not needed globally as "Questions".

Drag a Text box for "Additional notes" and place it where best suited.

Click Save. The template is now ready to use as a Templated Note in Curve Hero.
