Note
Each question can be modified, deleted, and duplicated. This can be prevented by removing the 'Manage Custom Fields' permission from each user.

Additional Notes
A short answer field for any additional notes.
Anesthesia
A yes/no question for if topical anesthesia was used, a select box for which anesthetic is chosen labeled 1-4, and a text box for the reaction and additional notes.
Assistant
A select box for assistants, labeled 1-3.
Consent signed
A required checkbox for consent signed.
Dental History
A select box containing the following options: Not been seen by a dentist prior to today, Has been seen but not in the last year, seen every 6 months.
Diet
A select box containing the following options: Bottled Water, Tap Water, Sugary Drinks, Fruits/Vegetables, Healthy Snacks, Candy/Cookies
Hygienist
A select box for HYG, labeled 1-3.
Main concern
A select box containing the following options: Check up, Teeth pain, Cleaning, Teeth sensitivity, Other. Below, a short answer requesting explanation if other is selected.
Medical History
A required yes/no question for if Medical History has been reviewed and updated.
Next Visit
A Short answer field for the next visit.
Observation
4 Columns with Age, Height, Weight, and Blood pressure. Below is a short answer for additional observation.
Patient Presents with
A select box containing the following options Mom, Dad, Grandma, Grandpa, Sibling, Legal Guardian, Care Giver, and Social Worker.
Patient Response
A Radio field containing the following options: Calm, Anxious, and Non compliant.
Post op instructions
A checkbox for 'Discussed Post Op Instructions'.
Provider
A select box for Provider, labeled 1-3.
Radiographs
A select box for radiographs containing the following options: 2 BWX, 4 BWX, Pano, PA(s), Other.
Sedation
A select box for sedation containing the following options: No Sedation, Nitrous Only, OCS, and IV Sedation.
Section 2: Uncategorized
Anesthetic Used
A short answer field for Anesthetic Used.
Calculus
A radio field for calculus containing the following options: Mild, Moderate, Severe.
Composite Restoration
3 Columns containing a short answer for Site, a Select box for Surface containing all surfaces, and a dropdown for material containing all possible materials.
Crown Prep
A short answer for Site, a dropdown for material containing all possible materials, and a yes/no for build up completed.
Diagnosis Reason
A Select box field containing the following options: Decay, Fracture, Other. Below, a short answer requesting explanation if other is selected.
Fluoride Varnish
A yes/no for if Fluoride Varnish was applied.
Health History Reviewed
A yes/no for if Health History was reviewed.
Oral Hygiene
A select box field containing the following options: Excellent, Good, Fair, Poor.
Radiographs Taken
A select box for radiographs containing the following options: 2 BWX, 4 BWX, Panoramic, Periapical, Full Mouth.
