This article demonstrates the steps to create a Note Template that can be used for patient appointments where a crown or multiple crowns are done.
About
Note Templates are different than Questions. Questions can be created first, which can make the process of creating Note Templates much easier. The examples used in this article are done by use of Questions that are already created. Create a set of Note Templates that fit the specific needs of the patients and appointments for your clinic rather than following this example exactly as it is laid out. This is intended to assist and give ideas so the end result fits the specific types of templates that will be helpful in the office. All questions and note components in this article are suggestions only, and are meant to be customized.

Section 1 - Creating a Note Template Category

Note Templates are created in Curve Forms. Click the Application Switcher button, then click Curve Forms.

The Curve Forms page opens. In the left sidebar, click the Note Templates icon.

The Note Templates page opens. There are two options from which to select, Questions and Templates. Click Templates.

The Note Categories page opens. Create a note category for the note template. Click Add Template Category.

Type the name for this category in the Template Category Name field. When finished, click Save.

The template category displays in the Template Category column.

Section 2 - Creating a Note Template with Questions

Click the More options menu on the desired category row. Click Add Template to Category.

The Template Builder page opens. Click the Template Name field, and type the name for this note template.

Note templates are built by pulling components or previously created questions from the Template menu into the Template Builder section.

- The Template menu consists of the Tool Box, and any Question categories the office has previously created. To learn more, click Building a Complex Question for Crowns for Note Templates.

In the Template menu click the desired Category to expand the available Questions beneath it.

To build a Template from existing Questions, click the desired Question, and drag and drop it into the Template Builder section.

The Question displays just as it was created in the Question Builder module. Hover over any of the components to display the action icons.

In the Template menu, click any Category to access the available Questions.

Click the Question and drag and drop it into the Template Builder section.

Continue adding Questions until all desired components are added.

Section 3 - Adding Additional Note Template Components

In the Template menu, click Tool Box to access component options.
Note: Components created directly in the Template will not be available to use in other Note Templates. If the component needs to be accessible elsewhere, consider creating it as a Question instead. Click Adding a Question for Note Templates in Curve Forms to learn more.

Click the desired Component in the Tool Box, and drag it into the Template Builder section.

Hover over the Component as needed, and click the Edit icon.

Edit the component so it displays and functions according to office requirements.

When all questions and components have been added and edited, click Save.
