This quick guide provides an overview of how to create a form, add a form to a patient's account, send a form to a patient, and also how to view a completed form.
About
When a clinic requires information from a patient, a form can be used to obtain this information. The patient can complete the form electronically or during their visit. The completed form can then be viewed in Curve Hero.
Included below are the links to quick guides that outline the forms process:
- Before a form can be added to a patient's account, the form must first be created in Curve Forms. For instructions regarding how to create a form, refer to the quick guide, Adding a Form Template in Curve Forms.
- There are a number of predefined fields that can be added to a form. If a custom field needs to be added to a form, it can be created in Curve Forms. For instructions regarding how to create a custom field, refer to the quick guide, Creating a Custom Field in Curve Forms.
- Forms that are stored in Curve Forms can be organized by creating form categories. For instructions regarding how to create a form category, refer to the quick guide, Adding a Form Category in Curve Forms.
- After a form has been created, it can be added to a patient's account. For instructions regarding how to add a form to a patient's account, refer to the quick guide, Adding a Form to a Patient's Account.
- If a form has been added to a patient's account, and the patient is visiting a clinic, the form can be opened for the patient who can then complete the form during their visit. For instructions regarding how to open a form for a patient to complete during a clinic visit, refer to the quick guide, Opening an Incomplete Form for a Patient to Complete.
- If one or more forms have been added to a patient's account and the patient will be completing the forms online, a link to the forms can be emailed or texted to the patient. - After a patient has completed a form, a PDF file of the form will be stored in Curve Hero. For instructions regarding how to view this file, refer to the quick guide, Opening a Completed Form in Curve Hero.

When a forms email reminder campaign is created in Curve
GRO, an email will be automatically sent to a patient anytime
an appointment is added or updated in Curve Hero. This email
will include a link that enables a patient to complete the form(s)
assigned to them. For instructions regarding how to create a
forms email reminder campaign in Curve GRO, refer to

Note: It is not necessary to create a campaign specific to forms. Rather, any reminder campaign can be created which includes the applicable forms template(s). In doing so, this can reduce the number of emails and/or text messages that are sent to the patients. - A template can be created for the email message that is used in a forms email reminder campaign. For instructions regarding how to create a forms email template in Curve GRO, refer to the quick guide, Creating a Forms Email Template.
- When an active forms email reminder campaign sends an email to a patient, the patient will receive an email which includes a link to their uncompleted and/or completed form(s). The patient will have the ability to complete the uncompleted form(s) prior to their visit.
