This quick guide outlines the steps required to create a form in Curve Forms.
About
When a form is created, it can be accessed in the Forms module within Curve Hero. The primary benefit of using forms is that it will expedite the process to create specific forms in Curve Hero. Form categories can also be created in the Forms section which provides the ability to organize forms as required.
Note
In order to access the Form Templates section in Curve Forms, the user must be assigned to a role in Curve Hero which contains the Manage form templates permission. For more information about permissions, refer to the quick guide, Understanding Curve Hero and Curve GRO Access Permissions.

Section 1 - Opening Curve Forms

In Curve Hero, click the Application switcher drop-down menu, then click Forms within the drop-down list.

Section 2 - Accessing the Forms Section

In the Sidebar menu, click Form Templates.

The Form Templates section opens and the form categories will display.

Section 3 - Adding a Form

In the Form Templates section, click the Add Form button.

The Add Forms section is displayed.

In the Form Name field, enter the form name.

To update which form category the form will be saved in, click the Category drop-down menu.

In the Category drop-down list, click the applicable form category.

Field element(s) can now be added. Options include Tool Box, Predefined Form Categories, and Custom Fields.
Note: The Custom Fields category will not display unless custom fields have been added into the database. Click Creating a Custom Field in Curve Forms to learn how to do this.

To add a field element, begin by clicking one of the form categories.

In the form category drop-down, locate the field element.

After locating, drag and drop the field element into the Form Builder. Ensure the row displays in blue before releasing.

The field element will display. The predefined fields cannot be edited. The blank fields (any that are in the Tool Box category) can be edited. Any of them can be made Required.
Note: Some of the fields, such as First Name - Patient, and Last Name - Patient will already be required by default.

In the dialog box, click the Required box, then click Ok.

A required field will be marked with a red asterisk.

To add additional field elements, repeat steps 7 to 9.

Note that predefined field elements can only be used once, except for the Initials component. This can be added as many times as is needed.

After adding all the elements, click the Save button.

The form template will be saved to the applicable category.

Section 4 - Previewing a Form

To view a preview, click the applicable form. The preview pane will display the form.
