This Quick Guide provides an overview on creating and using Form Packets.
About
Users are able to group multiple forms together into a "packet" and assign the whole group to a patient in one action without manually adding individual forms for each procedure or office action. Common uses may be creating a New Patient Paperwork form packet, an Oral Surgery and Extraction packet, or a Crown Consent Packet containing forms for Pre-Appointment Instructions, Crown Consent, and Post-Op Instructions. This allows offices the ability to streamline patient form completion.
Important Note: Since forms added as part of a form packet do not have assigned frequencies, completing forms that are part of a form packet do not override the assigned frequency (ex: 6 months) of the same required form outside of a form packet.
Example: An Allergies and Conditions form may be a required office form with a completion frequency of once every 6 months. If an additional Allergies and Conditions form is added to a form packet, a patient will need to also complete the new Allergies and Conditions form as part of the form packet, and completing it will not replace or update the completion date of the original Allergies and Conditions form required by the office every 6 months. The patient will have to update the required Allergies and Conditions form again when it is due, regardless if they have completed the form inside a form packet within the last 6 months.

Section 1 - Creating a Form Packet

From the App Switcher icon, click Forms.
2. Curve Forms will open in a new browser window. Click the Form Templates icon.
3. Click Form Packets.
4. The Form Packets page will display, featuring a search and filter bar, Add Packet button, and Form Packet table where users can view, edit, or delete existing form packets.
5. To create a form packet, click Add Packet.
6. The Add Packet page will display and begin the form packet process at Step 1: Packet Details. Click inside the Name field and type in the form packet's name.
7. Click inside the Description field and enter in a description of the form packet.
8. Click inside the Category field.
9. A dropdown menu of form categories will appear. Click desired form category.
10. Click Next at the bottom right of the page.
11. The Step 2: Form Selection page will display where users will select forms to include in the form packet.
12. To search for a specific form, click inside the Search field and type in a form name.
13. A list of related forms will display. Click inside a gray selection box to select the desired form(s). Selected forms will display a blue and white checkmark box once enabled.
14. To clear the search bar, click the reset filter icon. This will display all forms again.
15. A list of all forms will display. Click inside the gray selection boxes to select desired forms to include in the form packet.
16. Once all desired forms have been selected, click Next.
17. The Step 3: Settings and Review page will display. Here, users may delete a form from the form packet by clicking the trash can icon, or reorder forms by clicking a form, holding, and dragging to a new order position.
18. To save, click Save.
19. The new form packet will now display on the Form Packets page.
20. To edit or delete a form packet, click the more options icon at the end of the form packet row.
Note: Clicking Edit will bring users back to the Step 1: Packet Details page.
21. Clicking Delete will display a Delete Form Packet dialogue box, informing users that only the packet will be deleted, not the forms inside the packet. To delete, click Delete.
Section 2 - Form Packet Functionality

To access form packets from the patient's Sidekick, click Forms.
2. Click Add Form.
3. An Add Forms dialogue box will open. Scroll to find and click inside the gray selection box to select a desired form packet (once selected, the box will become a blue checkmark box).
Note: Form packets will display a stacked-paper icon next to the selection boxes.
4. With the correct form packet selected, click Add.
5. The forms inside the selected form packet will now display as individual forms at the bottom of the patient's forms list in the Sidekick (users may need to scroll if there are many preexisting forms).
Important Note: Since forms added as part of a form packet do not have assigned frequencies, completing forms that are part of a form packet do not override the same form outside of a form packet that has an assigned frequency date (ex: every 6 months).
Example: An Allergies and Conditions form may be an office's required form with a completion frequency of once every 6 months. If an additional Allergies and Conditions form is added to a form packet, a patient will need to also complete the new Allergies and Conditions form as part of the form packet, and completing it will not replace or update the completion date of the original Allergies and Conditions form required by the office every 6 months.
