Users can now eliminate the manual task of assigning form frequencies to patients, ensuring no office-specific or legal forms are accidentally missed. This feature provides an automated, streamlined solution to form assignments.

Section 1 - Important Considerations Using Forms Frequencies

For Existing Patients:
When an appointment is created or edited, any forms that will expire before the appointment date are automatically assigned to the patient for completion. If an incomplete version of the form is already assigned to the patient, a duplicate form will not be assigned.

For New Patients:
An appointment must first be created before any forms will be assigned to a new patient. Forms marked for New Patient are automatically assigned to the new patient.

For All Patients:
Patients will still only receive the forms with their Appointment Reminder emails/texts.

"Due" Label:
The form expires today or has already expired.

"Due Soon" Label:
The form will expire in the next 14 days.
Section 2- How To Set a Form Frequency

Click the app switcher icon.
2. Click the Forms app icon.
3. The Forms app will open. Click the Form Templates icon.
4. The Forms page will open. Click the more options icon of the desired form.
5. Click Edit from the dropdown menu.
6. The Edit page will open. Click Form Settings.
7. A Form Settings: Form Title dialog box will open with options available to auto-assign the form: auto assign for new patients, and auto-assign for a designated frequency. Click the gray checkmark box to select a form assignment.
8. To assign this form to new patients when a first appointment is scheduled, click to select Assign form to new patients. A blue checkmark will appear once a form has been selected.
9. To assign a frequency to this form, click the checkmark box to select Automatically assign to active patients every: (frequency).
Important Note: For offices transitioning from physical/paper forms to Curve's digital forms, instances where a patient may complete physical/paper forms in-office will also receive a notice to complete the digital form version as well in order to remain in compliance, particularly for forms like medical history and demographics. Patients will still need to complete the digital form versions, even if they recently completed the physical form. Forms will populate in the patient's Sidekick and be available for patient completion.
10. To change the frequency in months of how often a form needs to be updated by a patient, click inside the Frequency (Months) field.
11. Type in the new frequency number for the form. Users can also click the increase/decrease arrows to increase or decrease the frequency number.
12. Click Save.
Section 3- Viewing Form Frequencies
1. Under the Forms dropdown in a patient's Sidekick, forms will show a Due (form is due today or is already expired), Due Soon (form will expire in the next 14 days), or a Green Checkmark (form is up to date) indicator to display a form's due status.
2. To complete a due form, click the more options menu icon of a due form.
3. Click Open.
4. The form will open. Complete the fields inside the form, then click Complete Form.
Note: Mandatory fields with a red asterisk must be completed in order to complete and save a form.
5. The completed form will now display a green checkmark icon to indicate the form is now up to date.
6. A form's due status will also appear on the Add Forms page.
