This quick guide contains all the links to the tracking sheets which are to be used to track all the activity that happens in the clinic during the conversion days, or the period between the final data pull and when you go live in Curve Hero.
The information that is recorded on these sheets should be transferred into Curve as soon as your live Curve Hero database is delivered to you.
Tracking Sheets

Profile Tracking Sheet - Use this to track any new patients that are added, and any personal information that has changed since the last time an existing patient came in.
New Appointments Tracking Sheet - Use this to track all the new appointments that were scheduled during the conversion days, as well as any changes to existing appointments (such as provider or procedure changes).
Canceled, Missed, and Rescheduled Appointments Tracking Sheet - Use this to track all the appointments that were canceled, missed, or rescheduled for another day/time or operatory.
Insurance Updates Tracking Sheet - Use this to track any policy, plan or carrier changes that happed during the conversion period.
Clinical Data Tracking Sheet - Use this to track all treatment planning that is done during the conversion days, as well as the current day's completed procedures, clinical notes and associated provider. Additionally, track all the new patient charting history, including conditions and tooth statuses.
Billing Updates Tracking Sheet - Use this to track transactions that were done during the conversion days, including payments, adjustments, and credits.
