This article demonstrates how to build a Question component for filling material that can be added to Note Templates for fillings.
About
Questions are used as the building blocks of Note Templates. Create a set of questions that fit the specific needs of the patients and appointments for your clinic rather than following this example exactly as it is laid out. This is intended to be a guide only.
Note
There are various ways to create Questions for building Note Templates. This article demonstrates some options, but the office can select the question components they prefer. It can be helpful to open an existing QuickText template for fillings in Curve Hero to get ideas for creating the Questions in Curve Forms.

Section 1 - Creating a Question Category
1. Click the Application switcher drop down menu, and click Curve Forms.
2. Curve Forms opens in an additional tab. In the Sidebar click the Note Templates icon.
3. The Note Templates section opens. Click Questions to open the Questions module.
4. To create a new category, click Add Question Category.
Note: If the category has already been created, click Section 2.
5. Type the question category name into the Question Category Name field. Click Save when finished.
6. The Question Category just created displays in the column. Click the More options menu on the right.
Section 2 - Create a Question Component for Filling Material
1. Click Add Question to Category.
2. Click the Question Name field at the top left, and enter the name for this question.
3. In the Tool Box click Dropdown, and drag and drop it into the Question Builder Section.
4. Hover over the Dropdown component and click the Edit icon.
5. The Select Box Component window opens. Click the Label field, and type Material.
6. Click the Answer tab. Enter the possible filling materials in the Label fields.
7. Click Add another to enter additional material options.
8. Type the material option in the Label field.
9. Repeat until all materials are added, then click Save.
