This Quick Guide demonstrates how to create a Route Slips Report and utilize it to create printable route slips from Curve Hero.
About
Printed route slips are something many offices have used with other dental software platforms. Curve does not have route slips that are printable, but all the information contained on a route slip is easily accessed in the patient Sidekick, the patient appointment and the Snapshot report. With this information being so readily available in Curve, most offices find they do not need a printed route slip. This guide contains instructions on how to create them for offices who still want them.

Section 1 - Creating a Route Slip Report
- To create a Route Slip report, click the More menu, then click Reports.
- The Reports module opens. Click Create a New Report.
- In the dialog box that opens, click the first blank Name line.
- Type the name of the report, and hit Enter on the keyboard.
- Click the Category field. Click Appointment Reports in the dropdown menu that displays.
- Click Type. The report type dropdown menu appears. Click Appointment Reports.
- The columns dropdown list appears. Click the desired columns to include on patient route slips.
- First name
- Last name
- Appt time
- Appt procedures
- Home phone
- Mobile
- Default doctor
- Date of birth
- Age
- Medical alerts
- Referral source
- RP full name
- Total owing
- Insurance total
- Patient total
- Last payment date
- Last payment amount
- Primary carrier name
- Primary remaining individual benefits amount
- Sec carrier name
- Sec remaining individual benefits amount
- Family members
- Next appt date
- Next appt description and Clinic.
- When all the columns are selected, click Finish.
- The Route Slip Report opens.
- In the Filters section, click Add a filter.
- Click Column. The columns dropdown list appears. Select Appt. date.
- Click on date, and in the dropdown list that displays, click dynamic date.
- Click select dynamic date and select Today.
- To run a route slip report for the appointments by clinic, add another filter, and for the column select Clinic. Click Select, and select the desired clinic in the dropdown list.
- Click Apply filters.
- The report generates to display the appointments for the current day at the selected clinic. Click Save in the upper right corner.

Note: If the office does not convert their A/R, the route slip will not show the patient owing amount until after the patient has been seen for the first time. Any outstanding balance in the legacy software will not transfer to the route slip.
Note: This report is now accessible under Appointment Reports in the Reports module and can be accessed thereafter.
Section 2 - Downloading and Printing Route Slips

Note: The steps demonstrated in this section are done in Word. Curve does not have any affiliation with Word, nor is it responsible for documenting any updates or changes to the functionality demonstrated in these steps. The functions below can be done in another platform besides Word.
- The data on the route slip report can be downloaded to a CSV file. Click CSV. The report downloads to a CSV file.
- In Word, with the Route Slip Template open, click Mailings in the top menu.
- In the Mailings setting, click Select Recipients.
- Click Use an Existing List.
- The window opens to select a file. Click Downloads, then find and select the downloaded route slip CSV file and click Open.
- The File Conversion window opens. Click OK.
- Click Ok.
- In the Mailing setting in Word, click Finish and Merge.
- Click Edit Individual Documents in the dropdown list.
- A pop-up message displays how many records are created. Click Ok.
- Click Print. This sends the route slips to the printer.
Important note: Click Route Slip to download the Route Slip template. This will be used to convert the information into a printable format.
Note: See the Important Note in Section 2 to download the template.
Note: The message may indicate there are empty records. This can be disregarded. Click Ok as many times as this pop-up message appears.
